Step 11: In the “Is your place of birth out of India” section, select yes if the applicant’s place of birth is out of India otherwise no.
Step 12: In the “Village or town or City” section, enter the residential town of the applicant.
Step 13: In the “State” section, choose the state of the applicant from the dropdown.
Step 14: In the “District” section, choose the district of the applicant from the dropdown.
Step 15: In the “Marital Status” section, select the status of the applicant from the dropdown.
Step 16: In the “Citizenship of India by” section, choose the nature of the citizenship of the applicant.
Step 17: In the “PAN” section, enter the PAN details of the applicant.
Step 18: In the “Voter ID” section, enter the details of the Voter ID of the applicant.
Step 19: In the “Employment Type” section, enter the nature of employment of the applicant.
Step 20: In the “Either of your parent (in case of a minor)/ spouse, A Government Servant” section, select Yes if either of Applicant’s parent or spouse is a government servant otherwise no.
Step 21: In the “Educational Qualification” section, select the qualification of the applicant from the dropdown.
Step 22: In the “Are you eligible for non-ecr category” section, choose yes if the applicant is eligible for the non-ecr category select yes otherwise no.
Step 23: In the “Visible Distinguishable Mark” section, If the applicant has a distinguishable mark on his body, then select yes otherwise no.
Step 24: In the “Aadhaar Number” section, enter the Aadhaar number of the applicant.
Step 25: In “I, THE HOLDER OF ABOVE MENTIONED AADHAAR NUMBER, HEREBY GIVE MY CONSENT TO PASSPORT SAHAYAK TO OBTAIN MY AADHAAR NUMBER, NAME, AND FINGERPRINT/IRIS FOR AUTHENTICATION WITH UIDAI. I HAVE NO OBJECTION USING MY IDENTITY AND BIOMETRIC INFORMATION FOR VALIDATION WITH THE AADHAAR (CIDR) DATABASE ONLY FOR THE PURPOSE OF AUTHENTICATION.” section, select Agree.
Step 26: In the “Father’s given name (given name means first name followed by the middle name (if any))” section, type the first and middle name of the applicant’s father.
Step 27: In the “Surname” section, enter the surname of the father of the applicant.
Step 28: In the “Legal Guardian’s given name” section, enter the first and middle (if any) name of the legal guardian.
Step 29: In the “Surname” section, enter the surname of the guardian of the applicant.
Step 30: In the “Mother’s given name” section, enter the first and middle name( if any) of the applicant.
Step 31: In the “Surname” section, enter the surname of the mother of the applicant.
Step 32: In the “If the applicant is minor” section, check the box if the applicant is a minor.
Step 33: In the “Is your present address out of India” section, select yes if the applicant is outside India otherwise no.
Step 34: In the “House no and Street name” section, type in the house number and street name where the applicant resides.
Step 35: In the “Village or Town or City” section, type in the town of the applicant.
Step 36: In the “State” section, choose the residential state of the applicant from the dropdown.
Step 37: In the “District” section, choose the residential district of the applicant from the dropdown.
Step 38: In the “Police Station” section, choose the residential police station of the applicant from the dropdown.
Step 39: In the “PIN” section, type in the residential Pincode of the applicant.
Step 40: In the “Mobile Number” section, type in the mobile number of the applicant.
Step 41: In the “Telephone Number” section, type in the phone number of the applicant.
Step 42: In the “E-mail ID” section, enter the email address of the applicant.
Step 43: In the “Do you have a permanent address” section, choose yes or from the dropdown.
Step 44: In the “Name” section, enter the name of the emergency contact.
Step 45: In the “Address” section, enter the emergency address.
Step 46: Write the mobile number of emergency contact.
Step 47: In the “Telephone number” section, provide the emergency telephone number
Step 48:In the “E-mail ID” section, enter the emergency email address.
Step 49: In the “Have you ever held/hold any identity certificate” section, choose yes if you have ever held it or you hold it otherwise choose no.
Step 50: In the “Details of previous/current diplomatic/official passport” section, select available if you have the details otherwise select details not available.
Step 50: In “Have you ever applied for passport, but not issued” section, select yes or no from the available options.
Step 51: In “I OWE ALLEGIANCE TO THE SOVEREIGNTY, UNITY & INTEGRITY OF INDIA, AND HAVE NOT VOLUNTARILY ACQUIRED CITIZENSHIP OR TRAVEL DOCUMENT OF ANY OTHER COUNTRY. I HAVE NOT LOST, SURRENDERED OR BEEN DEPRIVED OF THE CITIZENSHIP OF INDIA. I HAVE NOT CONTRAVENED ANY OF THE CONDITIONS RELATING TO THE POSSESSION AND USE OF AN INDIAN PASSPORT.
